Enter the 3-digit campus IDs (up to 5 per line)
with a comma between.

In the example above campus 060 and 065 will
be combined into campus 062 and will be reported

These parameters must be numeric and can not
contain any decimal positions. The values entered
will be printed on report CR6341B in the correct place. The 1,2,3,4 refer to the column on the report that the amount will be printed. Values entered for column 1 and 3 must be ten digits long and contain leading zeros. Values entered for column 2 and 4 must be six digits long and contain leading zeros. COLUMN 1 is the expenditure for 9/10 month faculty in whole dollars. COLUMN 2 is the number of 9/10 month faculty covered. COLUMN 3 is the expenditure for 11/12 month faculty in whole dollars. COLUMN 4 is the number of 11/12 month faculty covered. |

The required social security tax rate year. Enter as YY (i.e., 08). |
| VEST-AFTER-5A VEST-AFTER-5B | From 1 to 20, two-character retirement plan codes separated by commas may be entered in either of these parameters. These retirement plans will be added to VESTED AFTER 5 YEARS on report CR6341B. All other retirement plans will be added to VESTED WITHIN 5 YEARS. |
